What do I need to pre-qualify?

Are you at least 21 years of age?

  • You must bring a valid California driver's license or state issued ID card.

Are you on probation or parole?

  • Unless you have a permission letter from your parole officer you may not apply. Felons can not apply.

Do you have a medical qualifying diagnosis for medical marijuana usage?

What do I need for the day of my appointment?

You must qualify for cannabis certification prior to your appointment.

  • You must bring with you, not faxed, medical provider's notes, prescription bottle, xray reports, etc. to document the diagnosis.
  • You will be seen by a Physician who will perform a physical examination.
